Позитронная эмиссионная томография как метод неинвазивной оценки миокардиального кровотока и коронарного резерва у пациентов с сердечно-сосудистой патологией (литературный обзор)
Настоящая статья посвящена анализу литературных сведений, посвященных изучению патологии коронарного кровообращения при различных сердечно-сосудистых заболеваниях на основании результатов но-зитронной эмиссионной томографии (ПЭТ), Расчет миокардиального кровотока и коронарного резерва в различных отделах левого желудочка обеспечивает корректную оценку гемодинамической значимости коронарного атеросклероза. При патологических состояниях, ассоциирующихся с высоким риском развития сердечно-сосудистых заболеваний, коронарная недостаточность в подавляющем большинстве случаев обусловлена нарушением вазорелаксирующей функции эндотелия. Неинвазивная диагностика нарушений коронарной микроциркуляции играет важную роль в стратификации риска кардиоваскулярных осложнений. Особое значение приобретает исследование ПЭТ для объективной оценки эффективности медикаментозной терапии, направленной на коррекцию артериальной гипертензии, дислипидемии, сахарного диабета, гипоэстрогенемии. Нарушение коронарной микроциркуляции у пациентов с гипертрофической и идиопатической дилатационной формами кардиомиопатии является независимым предиктором коронарных осложнений и ассоциируется с высоким риском внезапной смерти.
