Цель. Сравнить прямые медицинские затраты при подготовке пациентов с фибрилляцией предсердий (ФП) к проведению плановой кардиоверсии (КВ) при лечении варфарином и дабигатраномМатериал и методы. Проведено открытое нерандомизированное исследование прямых медицинских затрат (стоимость препарата; затраты на контроль международного нормализованного отношения (МНО) в условиях поликлиники; затраты на визиты к кардиологу при подготовке к КВ). Включено 62 пациента с персистирующей неклапанной ФП (продолжительность пароксизма аритмии более 48 ч), которые последовательно обратились за медицинской помощью и в отношении которых было принято решение о КВ в плановом порядке. 32 пациента получали варфарин, а 30 – дабигатран. По основным клиническим показателям и риску тромбоэмболических осложнений (ТЭО) по шкале CHA2DS2-VASc группы были сопоставимы.Результаты. Длительность от начала лечения до проведения КВ в группе дабигатрана составила 21±2 дней, а в группе варфарина 30,5±4,5 дней (p<0,05). В среднем затраты на визиты к специалисту составили 3720 руб. в группе варфарина и 744 руб. в группе дабигатрана (p<0,05), а затраты на препарат 53,63 руб. и 1172,01 руб. соответственно (p<0,05). Затраты на лабораторный контроль МНО в группе варфарина составили 3058 руб. Совокупные затраты на одного больного в среднем в группе варфарина составили 6831,63 руб., в группе дабигатрана – 1916,01 руб. (p<0,05).Заключение. В реальной клинической практике выбор дабигатрана для профилактики ТЭО у больных неклапанной ФП при подготовке к плановой КВ может существенно снизить прямые затраты на лечение по сравнению с варфарином. Применение дабигатрана уменьшает время от принятия решения о плановой КВ и начала антитромботической терапии до проведения КВ.
