Механическая поддержка кровообращения при чрескожном коронарном вмешательстве высокого риска
Чрескожное коронарное вмешательство высокого риска подразумевает технические трудности при выполнении реваскуляризации, нестабильность сердечного ритма и гемодинамики и возможные осложнения. Одним из способов обеспечения гемодинамической стабильности и расширения возможностей для реваскуляризации являются использование устройств механической поддержки кровообращения. На данный момент арсенал методов представлен следующими устройствами: внутриаортальная баллонная контрпульсация, Impella, TandemHeart и вено-артериальная мембранная оксигенация. В обзоре рассмотрены актуальные сообщения, данные регистров, рандомизированных и наблюдательных исследований, а также клинические протоколы, посвященные использованию этих устройств для механической поддержки кровообращения во время чрескожного коронарного вмешательства высокого риска.
