Стресс–эхокардиография в алгоритме диагностики острого коронарного синдрома без подъема сегмента ST низкого риска
В представленном обзоре литературы рассмотрены возможности стресс–ЭхоКГ как простого, неинвазивного, не сопряженного с лучевой нагрузкой метода для диагностики скрытых нарушений коронарного кровотока у пациентов с острым коронарным синдромом без подъема сегмента ST на электрокардиограмме низкого риска. Представлены возможности расширенного протокола стресс–ЭхоКГ, при котором стандартное выявление преходящих нарушений локальной сократимости, ассоциированных, как правило, с обструктивным поражением коронарных артерий, дополнено оценкой резерва частоты сердечных сокращений, коронарного резерва и другими параметрами. Подобный подход рассматривается как перспективный для получения более полной характеристики функции сердца при нагрузке и точного прогноза клинического случая, позволяющих определить тактику ведения пациента, не ограничивающуюся отбором для реваскуляризации миокарда.
