Цель. Изучить эффективность антиаритмических препаратов (ААП): пропафенона, соталола и верапамила для предупреждения рецидивов фибрилляции предсердий (ФП) и других предсердных тахиаритмий (ПТ) в раннем послеоперационном периоде катетерной изоляции устьев легочных вен у пациентов с пароксизмальной формой ФП.Материал и методы. В проспективное сравнительное открытое рандомизированное исследование с контролем включены 243 пациента и разделены на 4 группы. Больным 1 группы (n=61) был назначен верапамил ретард 240 мг/сут, 2 группы (n=62) – пропафенон 450 мг/сут,3 группы (n=60) – соталол 160 мг/сут. Пациентам 4 группы контроля (n=60) ААП не назначались. Для выявления аритмий в раннем послеоперационном периоде использовались дневник пациента, ЭКГ, суточное мониторирование ЭКГ и система подкожного кардиомонитора.Результаты. За первые 3 мес послеоперационного периода в группе 2 зарегистрировано 8,62±9,37 медикаментозных кардиоверсий против 13,24±10,77 в группе 1 (р=0,013) и против 11,93±12,02 в группе 3 (р=0,123). В группе 2 было проведено 0,40±1,03 электрических кардиоверсий против 1,016±1,74 в группе 1 (р=0,024) и против 1,033±1,52 в группе 3 (р=0,0096). Также в группе 2 отмечено 0,447±0,57 случаев госпитализаций против 0,684±0,73 в группе 1 (р=0,0012) и против 0,592±0,67 в группе 3 (р=0,074).Заключение: В группе пропафенона выявлена лучшая клиническая эффективность для профилактики рецидивов ПТ в раннем послеоперационном периоде.
