Поражения клапанов сердца вследствие лучевой терапии злокачественных новообразований

Актуальность. Лучевая терапия (ЛТ) играет важнейшую роль в онкологии, улучшая непосредственные и отдаленные результаты лечения целого ряда опухолей. К числу наиболее значимых для прогноза осложнений ЛТ относятся поражения клапанов сердца.Цель. Изучить варианты поражения клапанов, возникающие у больных, получавших лучевую терапию по поводу онкологических заболеваний. Пациенты и методы их обследования. Обследована группа больных, получавших в период с 1978 по 2002 г. химиолучевую терапию (ХЛТ) по поводу лимфомы Ходжкина (ЛХ) 2–4 стадий с поражением внутригрудных лимфоузлов: 71 пациент, 60 из которых не обращались ранее к кардиологу и были приглашены на обследование, 11 были госпитализированы в связи с клинически значимой сердечно-сосудистой патологией (ХСН, инфарктом миокарда, стенокардией, клапанным дефектом, АВ блокадой).Методы исследования: стандартное клинико-лабораторное обследование, спирометрию, суточное мониторирование ЭКГ, ЭхоКГ, у части пациентов – однофотонную эмиссионную томосцинтиграфию миокарда (ОЭКТ), и КТ органов грудной полости. У 60 боль‑ ных выполнили нагрузочный тест на тредмиле с ЭКГ контролем, у 18 – максимальный нагрузочный тест на тредмиле с газовым анализом – эргоспирометрию.Результаты и обсуждение. Патология клапанов выявлена в 49,3 % случаев, наиболее часто (у 46,5 %) встречалась митральная регургитация (МР), в первую очередь за счет МР 1 степени, которая не имела клинического значения. Патология аортального клапана (12,7 % пациентов), была представлена в основном нетяжелой регургитацией (11,3 %). Аортальный стеноз диагностирован у 4,2 % обследованных. В исследованной когорте пациентов выявлены преимущественно нетяжелые клапанные поражения. Кроме того, представлены примеры больных с клинически значимыми лучевыми поражениями клапанов.
