Пациенты с хронической сердечной недостаточностью со сниженной фракцией выброса левого желудочка (ХСНнФВ) представляют проблему здравоохранения, т.к. у некоторых из них, несмотря на соблюдение рекомендаций и получение базовой терапии в полном объеме, наблюдается ухудшение течения заболевания. Особенно высок риск развития неблагоприятных исходов у пациентов, которым требуется повторная госпитализация или оказание неотложной помощи в амбулаторных условиях. Им необходима терапия нового типа. Верицигуат — первый пероральный стимулятор растворимой гуанилатциклазы, одобренный для лечения взрослых с симптоматической ХСНнФВ и признаками клинического ухудшения. В этой статье обобщены основные результаты применения данного препарата. Рассматриваются эффективность и переносимость верицигуата, зависимость его действия от исходного состояния пациентов: наличия ишемической болезни сердца, фибрилляции предсердий, почечной недостаточности, уровня N-концевого пропептида натрийуретического гормона (В-типа) и длительности периода после госпитализации. Сообщается, что применение препарата снижает риск госпитализации по поводу ухудшения сердечной недостаточности и смерти от сердечно-сосудистых заболеваний у всех пациентов с ХСНнФВ независимо от рассмотренных параметров исходного состояния, однако не оказывает существенного влияния на качество жизни больных. Эти результаты могут быть полезны для понимания того, как лучше всего применять верицигуат у растущей популяции пациентов с ХСНнФВ с недавним ухудшением их состояния, несмотря на получение стандартной терапии, основанной на рекомендациях.
